Impact That Builds Better Lives

 

Recovery is more than completing a program. It’s finding stability, rebuilding confidence and creating a path forward.

 

At Houses of Hope, people with substance use and mental health conditions receive support through outpatient services, transitional living and residential treatment — with care designed to meet the real-life challenges they face every day.

 

In FY25, individuals across our programs showed measurable progress in daily functioning, sobriety, employment and connection to healthcare. Behind every number is a person working toward a healthier, more stable future.

 

Outpatient Services

Launched on July 1, 2025, outpatient services expanded access to recovery support for individuals needing flexible, ongoing care.

In the first six months:

•192 individuals served

 

A comprehensive outcomes analysis will be completed at the end of FY26 as the program continues to grow.

 

Transitional Living

Often referred to as halfway houses, our transitional living program helps men continue building stability after treatment in a structured, recovery-focused environment.

 

In FY25:

•166 referrals received

•123 individuals served

•Average stay: 107 days

 

Outcomes at Discharge:

•86% sober

•72% employed

•62% connected to a primary healthcare provider

•91–95% client satisfaction

 

Men served also showed measurable improvement in daily functioning, moving from serious to moderate impairment on the nationally recognized DLA-20 assessment.

 

Short-Term Residential Treatment

Our short-term residential treatment program serves individuals with complex needs, many entering care while facing homelessness, mental health instability or limited financial resources.

 

In FY25:

•1,481 referrals received

•717 individuals placed on a waitlist

•153 individuals served

 

Who We Served:

•77% experienced co-occurring substance use and mental health conditions

•22% were experiencing homelessness at admission

•97% relied on Medicaid or state behavioral health funding

 

Outcomes at Discharge:

Participants demonstrated meaningful progress in daily functioning and recovery stability, with 85–95% client satisfaction.

Funding Statement: For FY26, Houses of Hope is funded (3.48% or $185,958.90) from the Substance Abuse Prevention & Treatment Block Grant #B08TI087052 through the Substance Abuse and Mental Health Services Administration, contracted through Region 2 and Region 5 Systems and the Nebraska Department of Health and Human Services, Division of Behavioral Health. Additionally, funding from the Division of Behavioral Health (41.92% or $2,238,310), Medicaid (40.36% or $2,154,902) and other non-governmental sources (14.24% or $760,410) support our projects.
