Transitional Living Information

Qualifications to apply for the Transitional Living Program

 

Please answer the following questions:​

  1. Do you Identify as male?

  2. Are you age 19 or older?

  3. Are you on track to successfully complete an inpatient treatment program or graduated from one in the past 6 months?

  4. Do you have a drug & alcohol evaluation (completed in the last 60 days) that recommends a 3.1 level of care or halfway house?

  5. Do you have the ability to work 35+ hours a week? (you do not need to currently be employed)

  6. Are you willing to actively participate in an individualized treatment plan?

  7. Do you have the desire to maintain sobriety and abstain from all substances? (including over the counter substances such as kratom, CBD, etc.)    * Nicotine use is allowed inside designated areas

Program Information

 

Transitional Living treatment plans are individualized for each resident enrolled into the program.

During orientation, you will work with our clinical team to develop a treatment plan that both identifies your goals for treatment and transitioning afterwards.

​

The clinical staff offices are located on the same campus as the houses. This building is also where therapy & groups take place to ensure convenient on site clinical support. In addition our houses are monitored 24/7  by our behavioral health technicians to provide immediate assistance if needed.

 

The length of your stay will be determined by individual needs, the ability to meet goals and progress made during treatment.  

Each day, our goal is to move towards independence and a healthy life whether that be transitioning to a ¾-way house or alternative independent living once our program has been successfully completed.

Intake is an all day process so please plan for 6-8 hours. This takes place in our Administration building located at 1124 N Cotner Blvd.

You will be given a tour and all expectations will be explained. You may bring your personal items to move in on this day. Please take a glance at the lists of what to bring, what not to bring and guidelines for personal items. 

While you are a new resident you attend a daily Life Skills Group and Recovery Check-In group.

Residents also have the option to meet with our Case Manager who can assist residents in establishing providers and navigating needs. These include physical health, medication management, vision, alternative transportation solutions and requesting personal documentation.

Schedule

​

Clinical service hours vary between 9:00 a.m. and 8:00 p.m. to best meet client needs.

Daily schedules with vary for each resident.

Participation in house dinner is mandatory is you are not scheduled to work during that time.

All residents attend 4 recovery meetings a week in the community.

Other schedule requirements such as chores, curfew, visiting hours etc. will be gone over during intake.

Personal Items

​

Houses of Hope assumes no liability for personal property brought on the premises.

You will have adequate (but limited) space for personal items.

Your personal items MUST fit in spaces designated for your use.

Items that do NOT fit in designated spaces will not be allowed.

Items brought to the facility are subject to inspection and will be placed through the New Admission Protocol procedure. Any contraband items will be confiscated.​

What To Bring

Toiletries (toothpaste, toothbrush, soap, shampoo, shaving products, etc.)

Stationary and stamps

Towels and washcloths

AA/NA literature

Current prescribed and/or non-alcohol-based over the counter medications

Alarm clock

Small radio

Small personal items (photos, books, to make you feel at home)

Clothing appropriate to the season

ONE bicycle (if you have one)

ONE vehicle…and ONLY if you have a valid driver’s license, if the vehicle is licensed, has current insurance, and is in running condition. Please bring all documentation to verify including valid license, insurance card and registration.

Do NOT Bring

​

Alcohol-based toiletries—mouthwash, etc. (those items will be confiscated and disposed of)

TV, or desktop computer, monitor, printers

Component stereo equipment

Furniture

More than one bicycle

Personal Vehicle - To bring a vehicle on the property you must have a valid driver’s license, up to date registration and plates, current insurance, and is in running condition. Please bring all documentation to verify including valid license, insurance card and registration.

Note: Any personal property left at the time of discharge will be held for 14 days. During that time, attempts will be made to contact you or your emergency contact to retrieve those items. If not retrieved, these items will be disposed of.

Funding Statement: For FY25, Houses of Hope is funded (5.34% or $306,914.95) from the Substance Abuse Prevention & Treatment Block Grant #B08TI087052 through the Substance Abuse and Mental Health Services Administration, contracted through Region 2 and Region 5 Systems and the Nebraska Department of Health and Human Services, Division of Behavioral Health. Additionally, funding from the Division of Behavioral Health (34.91% or $2,005,809), Medicaid (50.03% or $2,875,000) and other non-governmental sources (9.72% or $558,410) support our projects.
